What is Tadalafil?

Tadalafil (also known by the brand name Cialis) is a treatment for erectile dysfunction (ED). Like most ED medications, it is a type of drug known as a phosphodiesterase type 5 (PDE-5) inhibitor. Tadalafil works in combination with sexual stimulation to help men achieve and maintain an erection sufficient for sexual activity.

How does Tadalafil work?

Tadalafil is a type of medicine called a PDE-5 inhibitor. Tadalafil works by relaxing blood vessels and increasing blood flow to the penis. This helps a man to achieve and maintain an erection when he is sexually aroused.

How effective is Tadalafil?

According to research, tadalafil improved erections in 67% of men taking a 10mg dose and 81% of men taking a 20mg dose.  Tadalafil is long-acting so is popular for those who wish to engage in sexual intercourse more than once per day, or those who don’t want to have to take a tablet prior to sexual activity. When you first start taking tadalafil, it can take a while to adjust, so it may not work for you straight away, but it is also normal to try several different ED medications before finding one that suits you.

Tadalafil and psychological ED

ED often has a psychological cause, such as anxiety or depression, in which case counselling is an effective treatment. This can take some time, and tadalafil may be suitable for use to treat ED in the meantime. However, it is important to be aware that this may mask the issue and counselling should nevertheless be pursued for effective, long-term results.

The active ingredient in Tadalafil is tadalafil.

Inactive ingredients

The inactive ingredients in tadalafil are lactose monohydrate, croscarmellose sodium, hypromellose, cellulose, microcrystalline, sorbitan stearate, magnesium stearate, titanium dioxide (E171), triacetin, talc, and iron oxide yellow (E172).

Which ingredients can cause an allergic reaction?

Tadalafil is rarely known to cause an allergic reaction in users. While it does contain lactose monohydrate, it is present in small amounts that should not cause any symptoms in those with lactose intolerance. If you do however have severe lactose intolerance, you might want to want to avoid this medication and use another erectile dysfunction medication.


Starting Tadalafil

The initial dose of tadalafil is typically 10mg, taken at least 30 minutes prior to sexual activity. This dose may be increased or decreased depending on your individual circumstances.

How to take Tadalafil?

Always take tadalafil as instructed by a doctor or pharmacist. Swallow one tablet with water at least 30 minutes before sexual activity. Tadalafil can be taken with or without food. Tadalafil may remain effective for up to 36 hours after taking the tablet.

For those who chose to take a lower “daily dose” of tadalafil rather than an “on demand” dose, one tablet should be taken once per day, with water.

Tadalafil dosage

The recommended starting dose for tadalafil is 10mg taken at least 30 minutes before sexual activity. If you feel this dose is too weak, your doctor may increase it to 20mg. For men who prefer spontaneous rather than planned sexual activity, a dose of 2.5mg or 5mg once daily may be prescribed instead.

How to get an erection whilst taking Tadalafil?

Tadalafil should be taken at least 30 minutes before engaging in sexual activity in order to enhance your ability to achieve and maintain an erection. You will trigger an erection in the usual way, through arousal and sexual stimulation.

Side Effects

Tadalafil side effects

Like all medications, tadalafil can cause side effects in some patients. If you experience any of the following rare but serious side effects, stop taking tadalafil and seek immediate medical care:

  • Allergic reactions including rash, facial swelling or difficulty breathing
  • Chest pain – if you experience chest pain, do not treat this using nitrates (including sublingual nitrates)
  • A prolonged and sometimes painful erection lasting longer than 4 hours (priapism)
  • Sudden loss of vision

Common side effects, affecting up to 1 in 10 people, include:

  • Headaches
  • Back pain
  • Muscle aches
  • Pain in arms and legs
  • Facial flushing
  • Nausea
  • Nasal congestion
  • Indigestion

Uncommon side effects can affect up to 1 in 100 people and include:

  • Dizziness (more frequent in men over the age of 75)
  • Stomachache
  • Diarrhoea (more frequent in men over the age of 65)
  • Nausea
  • Vomiting
  • Reflux
  • Blurred vision
  • Eye pain
  • Difficulty breathing
  • Blood in the urine
  • Prolonged erection
  • Pounding or increased heartbeat
  • High or low blood pressure
  • Nose bleeds
  • Ringing in the ears
  • Swelling of the hands, feet or ankles
  • Feeling tired

Rarely (up to 1 in 1,000 cases) the following side effects may occur:

  • Fainting
  • Seizure
  • Passing memory loss
  • Swelling of the eyelids
  • Red eyes
  • Sudden decrease or loss of hearing
  • Hives
  • Penile bleeding
  • Blood in the semen
  • Increased sweating
  • Migraine
  • Serious allergic reaction causing swelling of the face or throat
  • Serious skin rashes
  • A disorder affecting blood flow to the eyes
  • Irregular heartbeat
  • Angina
  • Heart attack
  • Stroke
  • Visual loss


Do NOT take tadalafil if you:

  • Are allergic to tadalafil or any of the other ingredients in this medication
  • Are taking any of the medications listed under “drug interactions”
  • Have serious heart disease or have suffered a heart attack within the last 90 days
  • Have had a stroke within the last 6 months
  • Have low blood pressure or uncontrolled high blood pressure
  • Have ever had a loss of vision due to non-arteritic anterior ischemic optic neuropathy (NAION)

Take particular care with tadalafil and inform your doctor if you have:

  • Any heart problems
  • Problems with blood pressure
  • Any hereditary eye disease
  • Sickle cell anaemia
  • Multiple myeloma
  • Leukaemia
  • Any deformation of your penis
  • A serious liver or kidney problem
  • Had pelvic surgery
  • Had all or part of your prostate gland removed

Drug interactions

Do NOT take Tadalafil with:

  • Any form of organic nitrate or nitric oxide donors such as amyl nitrate
  • Riociguat (used to treat pulmonary hypertension)
  • Doxazosin

Before taking Tadalafil, tell your doctor or pharmacist if you are taking any other medications, including those purchased over the counter without a prescription. In particular, tell your doctor if you are taking:

  • Alpha-blockers (commonly used for high blood pressure and prostate disorders)
  • Other medicines used to treat high blood pressure
  • A 5-alpha reductase inhibitor (used to treat benign prostatic hyperplasia)
  • Antifungals such as ketoconazole
  • Protease inhibitors for the treatment of HIV infection
  • Anticonvulsant medicines such as phenobarbital, phenytoin and carbamazepine
  • Rifampicin, erythromycin, clarithromycin or itraconazole
  • Other treatments for erectile dysfunction

Tadalafil and alcohol

Tadalafil can cause dizziness in some men and you should avoid drinking alcohol in excess during treatment. Alcohol can also contribute to problems with sexual performance, so it is best to limit your alcohol intake to experience the full effects of this medication.

Tadalafil and heavy machinery / driving

Tadalafil can cause dizziness in some users. Avoid driving or using heavy machinery until you’re sure you are not affected by this side effect.

Treatment Options

Lifestyle changes to improve ED

The root cause of erectile dysfunction is often psychological. Finding ways to reduce stress in your life can be extremely beneficial, as can maintaining a healthy lifestyle and exercising regularly. Anxiety may also contribute to the problem and can be further exacerbated by difficulties with sexual performance. Being open with your partner about your anxieties can often help you to relax. For advice on dealing with anxiety or depression, speak to a doctor or nurse.

If you are overweight, weight loss may help with erectile dysfunction; the most effective way to lose weight is with a combination of a healthy diet and regular exercise. Other lifestyle changes such as improving sleep pattern and reducing alcohol consumption can also help with ED.

Tadalafil vs Cialis

Tadalafil is the generic name for the branded drug Cialis. Both contain the active ingredient tadalafil and work in exactly the same way, although the generic version is significantly cheaper.

Tadalafil vs Cialis Daily

Tadalafil (brand name Cialis) is designed for one-time or occasional use and should not be taken daily. Cialis Daily contains a lower dose of tadalafil and is taken on a daily basis for long-term results.

Tadalafil vs other ED medications

The most common prescription treatments for ED in the UK are PDE-5 inhibitors. The earliest and most popular treatment for ED is sildenafil (brand name = Viagra), which is also available over the counter in the form of Viagra Connect. Tadalafil and Cialis differ from sildenafil in that they have a much longer duration of action, lasting up to 36 hours (compared to sildenafil which only lasts for 4–5 hours). Another PDE-5 inhibitor is vardenafil (brand name = Levitra) which functions in a similar way to sildenafil. The most recent treatment to be developed is avanafil (Spedra) which is faster-acting than tadalafil and can take effect in as little as 15 minutes.

Why doesn’t Tadalafil work for me?

Different ED treatments are better suited to different people, and it is not uncommon to try a few different treatments to begin with. If Tadalafil is not working for you, speak to your doctor about alternative treatments.

Will Tadalafil give me an erection for hours?

Tadalafil only works in combination with sexual stimulation, helping men achieve and maintain an erection when aroused. It does not normally produce an erection that persists in the absence of arousal and stimulation. A rare side effect is a prolonged and sometimes painful erection (priapism). If you experience this, stop taking tadalafil and seek medical attention.

Can Tadalafil lower blood pressure?

Tadalafil can have a lowering effect on blood pressure. This is not an issue for most men, but you should alert your doctor if you are already taking another blood pressure-lowering medication. Tadalafil is not licenced for the treatment of systemic high blood pressure but is sometimes used for the treatment of pulmonary arterial hypertension (high blood pressure in the arteries of the lungs).

Can Tadalafil treat enlarged prostate?
